Estimating fracture risk should be an integral part of bone health assessment. Multiple tools and strategies are available - some well validated, others more controversial but nonetheless potentially useful. We'll try to explore as many avenues as possible to get the fullest understanding of fracture risk for treatment recommendation.

Description
This presentation will first review the principle behind TBS, and why it correlates with other measures of trabecular bone microarchitecture and bone strength. We will review the effects of degenerative changes, fracture and other artifacts on TBS values. We will then review how the addition of TBS can alter fracture risk assessment and highlight clinical scenarios where the addition of TBS may alter fracture risk management decisions.

Dr. Shuhart was the founder and director of the Bone Health and Osteoporosis Center at Swedish Medical Group in Seattle, Washington. He is Past-President of the International Society for Clinical Densitometry (ISCD) 2019-2020. He is a co-author on the ISCD’s 2016 publication of “Best Practices” in the Journal of Clinical Densitometry, and was Co-Chair and Executive Author for the 2019 and 2023 ISCD Position Development Conferences.
Dr. Shuhart is now retired from clinical practice but continues to be active in teaching and training of residents, fellows and APC’s.
Dr. Shuhart received his medical degree from Dartmouth Medical School in 1987, and his Masters in Health Administration from the University of Washington in 2009. He is board certified in Family Medicine since 1990.

Dr. Schousboe is a health services researcher at HealthPartners Institute and past director of the bone densitometry center of Park Nicollet Clinic, a large integrated health care delivery system the Twin Cities, Minnesota. He is a Past President of the International Society of Clinical Densitometry (ISCD) and a Fellow of the American Society of Bone and Mineral Research (ASBMR). During a 20 year research career, he has been an author on over 250 peer-reviewed publications. His contributions to science include studies of the accuracy, prevalence, clinical impact, and cost-effectiveness of vertebral fracture assessment, identification and prognostic significance of abdominal aortic calcification identification on lateral spine bone density images, trabecular bone score in older men, and associations of fractures, frailty, physical performance, sleep disturbances, and cognitive impairment with total health care costs.
